In this song Bruce talks about being a man who had lost a family member in the 9/11 attacks. These lyrics could also be interpreted as a man who was a firefighter or police officer that helped out after these attacks.
He talks about how you can buy him a drink or shake his hand but he will still be the "nothing man". He will still not have his family back. He says "say you understand" as in saying please understand that the pain he is suffering is unimaginable.
